Educational Forms will be available at Registration.  The forms must be signed by a room monitor at the end of each workshop/keynote presentation.  The form includes a copy for the participant and one that must be returned to Registration at the end of the conference to send a certificate of attendance with the hours.

 

Vocational Rehabilitation Association of Canada

VRA members (RRP, CCRC) can complete.  Limited copies will be available at the registration desk.  Please download the form in advance of the conference.

Certified Psychiatric Rehabilitation Practitioner (CPRP)

Educational Forms will be available at Registration.  The forms must be signed by a room monitor at the end of the workshop/keynote presentation.  When CPRPs renew their 3rd year membership, the Commission sends out a package to be completed. There is a section to complete for your continuing ed hours. 

The education hours are applicable if the program falls within one of the following domains:
